# Medicare Home Health Care: What's Covered and How to Qualify

Medicare home health benefits allow you to receive skilled medical care in your home instead of a facility. For many beneficiaries recovering from illness, injury, or surgery, home health provides essential care in a comfortable setting—often at no cost to you.

What Medicare Home Health Covers

Skilled Nursing Care

  • Wound care and dressing changes
  • IV therapy and injections
  • Medication management
  • Monitoring vital signs
  • Teaching disease management

Physical Therapy

  • Strength and mobility exercises
  • Balance training
  • Pain management techniques
  • Transfer and walking assistance
  • Home safety assessments

Occupational Therapy

  • Activities of daily living training
  • Adaptive equipment instruction
  • Home modification recommendations
  • Energy conservation techniques

Speech-Language Pathology

  • Swallowing therapy
  • Speech rehabilitation
  • Cognitive-linguistic therapy
  • Communication strategies

Medical Social Services

  • Counseling for adjustment to illness
  • Community resource connections
  • Care coordination
  • Family support

Home Health Aide Services

  • Bathing and grooming assistance
  • Dressing help
  • Medication reminders
  • Light housekeeping related to care

Important: Aide services are only covered when you're also receiving skilled nursing or therapy.

Eligibility Requirements

To qualify for Medicare home health, you must meet ALL of these criteria:

1. Homebound Status

You're considered "homebound" if leaving home:
  • Requires considerable effort
  • Requires assistance from another person or medical equipment
  • Is medically inadvisable

You can still leave home for:

  • Medical appointments
  • Religious services
  • Adult day programs
  • Occasional trips (haircut, family event)

2. Skilled Care Need

You must need at least one of:
  • Skilled nursing care
  • Physical therapy
  • Speech-language pathology

Occupational therapy alone doesn't qualify you initially, but can continue care once established.

3. Doctor's Orders

A physician must:
  • Certify you need home health services
  • Establish a plan of care
  • Review and recertify periodically

4. Medicare-Certified Agency

Services must come from a Medicare-certified home health agency.

Cost to You

Original Medicare:

  • $0 for home health services
  • 20% coinsurance for durable medical equipment

Medicare Advantage:

  • Check your plan's specific cost-sharing
  • Many plans cover home health at $0
  • Some may have copays for certain services

How to Access Home Health Services

Step 1: Get a Referral

Talk to your doctor about whether home health is appropriate. They can refer you to a Medicare-certified agency.

Step 2: Initial Assessment

A home health nurse or therapist visits to assess your needs and develop a care plan.

Step 3: Care Begins

Services start based on the physician-approved plan of care.

Step 4: Ongoing Review

Your doctor reviews and recertifies your need periodically (every 60 days).

What Home Health Doesn't Cover

  • 24-hour care
  • Homemaker services alone (cooking, cleaning)
  • Personal care alone (bathing, dressing) without skilled need
  • Meal delivery
  • Custodial care

For these services, you may need to explore other options like private pay, Medicaid, or long-term care insurance.

Home Health vs. Other Care Options

Skilled Nursing Facility (SNF):

  • Short-term rehabilitation after hospitalization
  • 24-hour skilled care available
  • Medicare covers up to 100 days after qualifying hospital stay

Home Health:

  • Intermittent skilled care at home
  • You maintain independence
  • No prior hospitalization required

Long-Term Care:

  • Ongoing custodial care
  • Not covered by Medicare
  • Requires private pay, Medicaid, or long-term care insurance

Tips for Getting the Most from Home Health

1. Be honest about limitations - Accurate homebound assessment ensures coverage 2. Participate actively - Follow therapy exercises between visits 3. Communicate with your team - Report changes in condition 4. Ask questions - Understand your care plan and goals 5. Know your rights - You can choose your home health agency

Medicare Advantage Home Health Benefits

Some MA plans offer additional home health benefits:

  • More flexible homebound definitions
  • Additional aide hours
  • Telehealth check-ins
  • Transportation assistance
  • Meal delivery programs

Review your MA plan's benefits for any enhanced home health coverage.
